For Doctors and Professionals:

Please call Dr. Arnold’s assistant, Laura McGinley, at (303) 989 2086 or send a secure fax with the referral form to (720) 441-0480 if you wish to refer someone for neuropsychological assessment or psychotherapy. Please clearly state whether the referral is for neuropsychological evaluation or psychotherapy. Please also send relevant medical records and history for review, including cranial MRI, CT, EEG, and blood lab results.

For Patients or Individuals Seeking Services:

Please call Dr. Arnold’s assistant, Laura McGinley, at (303) 989 2086 to schedule an appointment. Neuropsychological assessment most often requires a referral from a doctor – please call Laura if you require assistance obtaining this.

 

Payment Options:

Dr. Arnold accepts private pay and insurance. She accepts the following insurances, but please note that it is your responsibility to contact your insurance company and to verify that the services will be covered. It will be your responsibility to pay for whatever your insurance company does not cover.
